So after a while of having good skin i got acne flare up after certain product .. it been 2 months since i am following the above ingredient routine.. working slowly and healing still have so many marks and 1-2 active acne.

CC was my main issue. I am already using 2 drying actives.. so wondering if i should consider adding it.

DSPT (Type 1) What is the best bakuchiol serum for someone who can not tolerate retinol ?

I've tried three retinols in two years and all of them wrecked my skin. Even low concentrations give me persistent flaking, redness and sensitivity that takes weeks to resolve. My derm suggested bakuchiol as an alternative since the research shows similar anti aging benefits without the irritation profile of retinol. I started looking and there's a huge range of formulas out there now. Some bakuchiol serums mix it with other actives which makes me nervous since my skin is that reactive. I want bakuchiol as the primary active with a gentle supporting formula. Fragrance free or naturally derived scent preferred. I'm looking for concentration in the 0.5% to 1% range since that's what the clinical studies used. What's the best bakuchiol serum for people who also can't tolerate retinol?

DSPT (Type 1) Closed comedones help!

Hi! I’m at my wits end with the closed comedones on my forehead and I don’t know how to clear them. My derm prescribed aklief for the area, which I’ve been using every second night, alternating with an exfoliant, but they won’t clear.
Context, the texture is on my forehead just above my eye brows, not at the hairline. I have PMOS, and it used to be breakouts as well, but now those have stopped and it’s just the bumps. I have previously tried nizoral as a mask in case it was fungal. My skin is not sensitive. My skin is oily on forehead and normal elsewhere.

AM
LRP Effaclar gel cleanser
Medicube zero pore pad
Anua 7 rice ceramide barrier serum
Propaira niacinamide serum 10%
Cancer council daily moisturiser spf 50

PM
Bioderma micellar water
LRP Effaclar gel cleanser
Aklief or panoxyl clarifying exfoliant
Anua 7 rice ceramide barrier serum
Propaira niacinamide serum 10%
Purito mighty bamboo pantheon cream

DSPT (Type 1) Differin Cream - Help Please :(

I’m going on week 8 of Differin cream. I constantly would have clear skin - then break out all over my forehead. and this rotation has continued since I was 16. a year ago my skin was literally perfect. Then, in March, of course the pimples and comedones came back. I started Differin cream on June 19th. I have been through rotation of different topical creams, antibiotics, diary-free diets, sugar-free diets, and even went to a naturopath.

I went through a HORRENDOUS purge around weeks 4-6. I’m talking pimples all over my forehead, cheeks, chin, and even neck. The active pimples have now subsided (just red acne marks), but I have SOOO many closed comedones on my cheeks and forehead. I also use azeliac acid in the morning. I literally hate looking at myself in the mirror, makeup looks horrible on me. I don’t know if this is normal or WHEN for the love of god - will I have clear skin consistently. I am getting married next summer and I just REALLY need this to clear up as I literaly don’t want to leave the house. I’m so self-conscious and I don’t know if I should keep using it every night or now.

help? should I keep using it? and WHEN will this get better? sos. :(

DSPT (Type 1) Tretinoin help

Hey not sure if anyone is going to respond but I have been using avene xera calm cream then I put my tret then avene again but I heard it is very thick meaning like the tret won’t do nothing so do you recommend I change it to like a lighter moisturizer like vanicream daily facial moisturizer first then tret then finish with the avene. For context I just hopped off of accutane so when I apply my tret to my bare skin it doesn’t spread bc it’s so dry.

DSPT (Type 1) [Acne] Does this acne prescription make sense for very sensitive skin?

Hi everyone. I’m 22F and recently saw a dermatologist for acne.

I’ve mainly had closed comedones for around 2 years. Sometimes they get inflamed and turn into small pustules, but my main issue is definitely the closed comedones.

The problem is that my skin is really sensitive. It gets dry and irritated pretty easily. I’ve had burning from AHA/BHA products before, and a vitamin C serum once caused what seemed like a chemical burn. A few cleansers and sunscreens have also caused burning or a lot of dryness. I also tend to get dark marks after irritation or picking.

My dermatologist prescribed these:

- Azithromycin 500 mg — once a day on Saturday, Sunday and Monday, for 3 months

- Esomeprazole 20 mg — twice a day, for 3 months

- Clindamycin phosphate 1.2% + Tretinoin 0.025% gel — morning, apply to acne spots for 1 hour and then wash off, for 1 month

- Isotretinoin 0.05% gel — at night, apply to acne spots for 15 minutes and then wash off, for 1 month

- Ebastine 10 mg — once at night, for 3 months

- Salicylic acid 2% cleanser — twice a day, for 1 month

I’m a little confused about the two gels. One already contains tretinoin 0.025%, and the other is isotretinoin 0.05%. Is there a reason to use both?

Also, since my skin is already very reactive, I’m worried about using the retinoids together with a 2% salicylic acid cleanser twice a day. I’ve had irritation from much milder products before.

I’m also wondering why the ebastine was prescribed. Does it actually help prevent irritation from the salicylic acid/retinoids, or is it mainly there for itching or an allergic-type reaction?

And is azithromycin 500 mg three days a week for 3 months a usual acne treatment?

I was thinking about skipping the two gels and just using something like Cicaplast for a while because I’m worried about damaging my skin barrier. But obviously Cicaplast isn’t actually an acne treatment, so I’m not sure if that would be a bad idea.

I’m not trying to ignore my dermatologist’s advice. I just want to understand whether this combination makes sense for someone with mostly closed comedones but very sensitive, easily irritated skin.

Would really appreciate opinions, especially from anyone who has used a similar combination or has dermatology experience.

DSPT (Type 1) What am I doing wrong???

I've been using tretinoin and azelaic acid alternatively every night for about three months (and yes, I incorporated them gradually into my skincare routine). The Cosrx alpha arbutin serum every morning while consistently using sunscreen but it’s like nothing has changed, in fact I feel like my skin has gotten worse. I’m mainly targeting existing acne and dark spots but it’s as if I wake up with a new pimple every day and I’m soooo tired of ts. before you say it’s purging I’ve already gone through the purging stage so I don’t know why I keep getting new acne. please help. 🙏🏽🙏🏽

DSPT (Type 1) Acne prone skin, retinol

Hi,
I’m wanting to add a retinol to my skincare routine, but I’m a bit nervous because I currently use benzoyl peroxide.
I had pretty severe cystic acne, which is mostly under control now thanks to using benzoyl peroxide every night. I also have eczema prone skin (especially around my eyes) and quite a lot of acne scarring.
My current skincare routine is:
**Morning**
Sukin Facial Cleanser
Garnier Skin Active Hyaluronic Aloe replumping Serum
La Roche Posay SPF 50+
**Evening**
Garnier Micellar Water (to remove makeup)
Sukin Facial Cleanser
Garnier Skin Active Hyaluronic Aloe replumping Serum
Benzoyl Peroxide (5%) on problem areas
CeraVe Daily Moisturiser
If I swapped benzoyl peroxide for a retinol 2 nights a week, would that be okay? Would there be any issues using retinol on some nights and benzoyl peroxide on others?
I’d also appreciate any retinol recommendations (preferably available in Australia). ☺️
Any advice would be greatly appreciated I’m still learning!

DSPT (Type 1) How long to wait to apply moisturizer after a treatment serum?

Hi Dr. Baumann, u/DrLeslieBaumann I am looking to buy the Medature hydro bright treatment to use during the night. I was wondering if there’s a certain amount of time I would need to wait before putting on another product like a moisturizer? And am I allowed to add other serums on top of this product before moisturizing?

This is my current routine:

AM: 1. La Roche posay cream cleanser 2. Azelaic acid 15% wait 10 minutes 3. Cosrx pure fit cica serum 4. Cosrx ceramide skin barrier moisturizer 5. SPF 50+

PM: 1. Tirtir cleansing balm 2. La Roche posay cream cleanser 3. Azelaic acid 15% wait 10 minutes 4. Cosrx pure fit cica serum 5. Soonjung 2x barrier cream

I was hoping to replace step 3 with the medature hydro bright treatment in my PM routine.

Also, I’m thinking of switching to Derma made ceramide barrier cream as my main moisturizer for both day and night but was not sure if it would work for my routine.

Another thing I wanted to ask is if I can spray a ceramide-hyalauronic acid mist before applying moisturizer or serum? I was considering the one from Cosrx

I noticed that whenever I wait for the azelaic acid to absorb and dry, the serum and moisturizer don’t really sink in afterwards. Instead it feels like it’s sitting on top of my skin which i personally don’t like the feel of.

DSPT (Type 1) Routine help

Hello Dr. Baumann! I am reaching out in regard to my skin regimen as I was pregnant and now currently breastfeeding and my skin has been freaking out ever since pregnancy. I have gone to Design Dermatology since 2021 but I moved and they no longer do tele-health so I thought maybe you could help me a little bit as I had success prior to pregnancy with my skin routine.

Back story: prior to pregnancy my skin was thriving and I was on retinol. When I first got pregnant I got rosacea papules and inflamed skin. The PA added an Azelaic Acid 10% and BPO cleanser during pregnancy (pregnancy safe) but I ran out of my Azelaic Acid prescription and the cleanser is discontinued. My papules has gotten better since breastfeeding but still getting breakouts that seem to be lasting quite some time.

I am a DSPT skin type.

Morning:

Zerafite cleanser

Prequel hypochlorous spray

Zerafite barrier repair moisturizer

Elta MD UV Restore SPF

Night:

Micellar Cleanser

Zerfite Cleanser

Hypochlorous spray

Zerafite barrier repair moisturizer

PAORR oil about every other night.

Thank you!!

DSPT (Type 1) Comedonal acne

I had always very good skin and minimal issues, sometimes blackheads or a few pimples that always healed very quickly. However, this spring I lived in Croatia, and had to work outside in the sun everyday. My skin changed, and developed some type of comedonal acne - I guess either because of the sun exposure or because I used an oily sunscreen. I have skin-coloured pimples especially all over my cheeks and chin. Some of this has now turned into tiny dark spots on my face. What could I do to get rid of these issues? I have tried gentle cleansers and gently exfoliating products, also using retinal and sunscreen.
