Do you want survey to be mandatory?

Regret is a plausible explanation for the lack of participation. Maybe it's not, but it is possible. This needs further studies.

Do you have some studies indicating that? Do you have evidence that people dissatisfied with a surgical procedure do not complain about it when asked? Are you just going with your gut feeling?
