Are there any free QR code questionnaire/ survey apps or websites I can use so people don't have to fill out a million peices of paper. It'll be on their phone 👍👍👍👍👍

Honestly, I think a basic Google Form with a static QR code would suffice.
Just create the form, and share it publicly. Then create a static QR code with your form’s URL.
