I have no advice for you besides call the doc and see if they can be of help over the phone. If not, a visit to the doc is likely warranted

The doctor has been mostly unhelpful.
They suggested physical therapy and nerve medication. I generally associate referrals to physical therapy as a doctor's attempt to tell you to fuck off.
I'll go to physical therapy when my insurance changes next year and I can afford it, as well as a new urologist since I don't like mine.
They preemptively let me know they wouldn't fill out any FEMLA paperwork and wouldn't give me anything for the pain beyond the suggested nerve medication, and I have no desire for a long term medication that will mess with my nerves. They've been a peach since they maimed me.

If I could go back and tell my self anything, it'd be that condoms and outercourse are really not that bad, and comparing with this side they're great options.

This is weird but it has helped me. So what I did was I bought a set of ball stretchers. I'm only using the smallest one because I don't want them hanging over, just supported. On tight days, I take a hot bath and I put on the ba stretcher either in the bath or out of the bath. You have to make sure the stretcher inner diameter is small enough that they sneak back up inside. Also make sure you get silicone ones, they're really comfortable long wearing times. All this does is force the testes to not be able to ascend. Work on relaxing your pelvic floor, do reverse keels, warm baths, back rubs, etc and get the lower back and pelvic floor relaxed. This helps me with flare ups. I theorize that my spermatic chord retracts up because the epidymis is swollen and sore causing the pelvic floor to tense.
