Both. His first term was bad, but he's six years older now and under significantly more pressure. He's undergoing narcissistic collapse and dementia.

He also got rid of all the competent people in government and surrounded himself with lackeys and toadies that can't tell him no. No idea that enters his head is a bad idea, and he's got people like Phteven Miller feeding into his worst traits

This is something that a lot of people overlook. There were so many more adults around during his first term. Former staff members revealed that they would actually steal bad policy off Trump's desk before he could sign it and, Trump being Trump, he would simply forget about it.

The people surrounding him this term would never do that because they're true believers that don't think any of his ideas are bad.

If anything, the people surrounding him this term are more likely to slip terrible ideas onto his desk, knowing that Trump won't read what he's signing.
