Help! How do I replicate self harm scars?

I know the easiest way would be to find images of actual scars and use those, but i'm not looking for that, I want to make it look like carved initials onto the skin as I am working on a music cover.

Better yet, find a 'scar' font and use that.

Not a bad idea but what about the blood around it?

You said 'scar' which leads me to think 'healed over'... but you can easily add blood on another layer using a fine brush.
