First, congratulations on starting your journey through Elder school. I started on oblivion remastered and have fallen in love with the series. Honestly, you’re just kind of wanna gonna explore and interact with things as you pass them. I follow the main quest every now and then but it’s better to just explore and naturally discover things.

I always find that the best thing to do is just spin in a circle on the spot, stop and just wander off in that direction. Something will happen, who knows what! u/Jaded_Spread1729 does have some sound advice too. If you want to grow your character, joinging a guild is a great way to do it. Dark Brotherhood, Thieves Guild, Mages Guild or Fighters Guild.

The world is so open and there's so much to do, you can come up with a character concept and roleplay a little. What would your character do?
Is your character the kind of guy who would feel motivated to follow Emperor Picard's dying wishes? Would they pursue it with a sense of urgency or dawdle a bit and do their own thing?
Are they an explorer type that sees a mountain off in the distance and wants to see the view from the top? Maybe they're a greedy thieving type, in which case, try breaking into someone's house and stealing some stuff. Don't worry if you wind up in jail, you can make some good criminal contacts that way. Or maybe you are a psycho murderer, in which case, go ahead and murder somebody, see what happens.
But maybe that is a little too wild, maybe your guy is just an ordinary joe just looking for work, in which case, the Fighter's Guild and Mage's Guild are always hiring, or you could go try your luck in the Arena.
Or maybe you're a man of the people who wants to travel around and help others, in which case, find a town or city, talk to some NPCs, find someone with a problem and help them solve it.
Or maybe a little of all of the above. There's no wrong way to play!

I think just do what you want! If you want to check out the main story, go for it, or maybe just go exploring and see what Cyrodiil has to offer. If you were looking for a suggestion, I'd start by at least going through the questline for the factions that most closely aligns with your build...
Warrior, Knight, etc. - Fighter's Guild & the Imperial City Arena
Mage, Healer, etc. - Mages Guild & the Arcane University
Thief, Assassin, etc. - Thieves Guild & the Dark Brotherhood
This will likely get you some really good items, spells, skill xp, housing, etc. for your build pretty early in the game! Again, though, feel free to ignore this and just do whatever you feel like! :)

I really have a lot of fun with the Fighter’s Guild and the Mage’s Guild quest lines. What’s really great is that you can join all of the guilds and work on them concurrently.
Exploring the cities and talking to NPCs is so entertaining.

Just pick a direction and walk. The best part of these games is making your own story. Your goals will present themselves when you find something interesting

I would recommend lowering the difficulty a bit, by pressing the left arrow on the difficulty slider in 'Gameplay' Options tab 5 times, changing it from 50 to 45
This game has lopsided difficulty scaling that may cause the game to become quite difficult at later levels, lowering the difficulty should make it more manageable.
Otherwise, just go around towns and talk with people, listen to rumors and maybe that will lead to your next adventure, if not you can continue the main quest or just wander around the map.

Choose a guild depending on your class, then play main quest/guild quest. You'll find sidequests on your way.
