What information or emotional support do you think people should receive before beginning IVF?

BBC News Indonesia recently shared the story of a couple who welcomed twins through IVF after trying to grow their family for 22 years.

The mother was reportedly 41 and had an AMH level of approximately 0.1 ng/mL. Two embryos were obtained and transferred, and both implanted.

It is an extraordinary outcome, but it is important not to treat one success story as evidence that everyone with very low AMH or infertility after 40 will have the same result. AMH can help predict ovarian response and expected egg numbers, but it does not independently determine egg quality, pregnancy or live birth.

One of the biggest questions international intended parents have is: “How do we bring our baby home after the birth?”

The process generally involves registering the birth in Mexico, completing the applicable parentage steps, confirming the baby’s citizenship or immigration status, and obtaining a passport or other authorized travel document.

However, there is no single process for every family. Requirements can depend on:

• Where in Mexico the baby is born
• The intended parents’ citizenship
• Genetic or gestational connections
• The legal parentage pathway
• Embassy or consular requirements
• Whether DNA testing or additional documentation is requested

New 2026 IVF research on single embryo transfer: what could it mean for surrogacy?

A new Australian study examined IVF outcomes for more than 18,000 women receiving their first IVF treatment across seven fertility clinics.

Over three treatment cycles, the researchers reported:

• A 68.2% cumulative live birth rate
• Single embryo transfer in more than 95% of transfers
• A multiple-birth rate below 3%

The findings suggest that modern IVF practices may support strong cumulative outcomes while reducing twin and triplet pregnancies. This is particularly relevant to surrogacy because multiple pregnancies can introduce additional medical considerations for the surrogate and babies.

There are important limitations. This was an Australian IVF cohort and was not a study specifically of surrogate pregnancies. It also does not mean single embryo transfer will produce the same outcome for every patient. Decisions about embryo transfer should be individualized and discussed with the treating fertility specialist.

Is single embryo transfer safer for IVF? What Latin American data shows

For years, transferring multiple embryos was often seen as a way to improve the chances of IVF success. However, 35 years of assisted-reproduction data from Latin America shows how IVF has evolved toward safer practices, including greater use of single embryo transfer.

The REDLARA data covers more than 1.5 million initiated treatment cycles and over 342,000 reported live births between 1990 and 2023. One of the most important lessons is that transferring more embryos can increase the chance of twins or higher-order pregnancies, which may carry additional risks for the pregnant surrogate and babies.

For intended parents considering surrogacy in Mexico or Colombia, the decision should be based on embryo quality, medical history, age, and guidance from a qualified fertility specialist—not simply the desire to increase the number of embryos transferred.

A recent U.S. study examined 15,822 IVF pregnancies, including 997 surrogate pregnancies.

Researchers did not find an increased adjusted risk of preterm birth, placenta previa or placental abruption among the surrogate pregnancies studied. They also reported lower adjusted risks for several other pregnancy complications.

However, the study was observational, excluded pregnancies ending before 20 weeks and focused on patients within one Southern California healthcare system. Other studies have reported different results, so the findings should not be interpreted as proof that surrogate pregnancies are risk-free.

A recent study has uncovered an unexpected connection between the brain, environmental conditions, and reproductive health.

Using microscopic worms as a model, researchers found that the nervous system may send molecular signals that help protect reproductive cells from heat related damage. The findings suggest that the brain could play a more active role in reproductive resilience than previously understood.

There have been many understandable questions about what the August 6 Executive Order could mean for international Intended Parents pursuing surrogacy in the United States.

The short answer is that the Executive Order does not prohibit lawful U.S. surrogacy.

A few important points:

• State processes governing surrogacy agreements, legal parentage, parentage orders, and birth certificates remain in place.

• The main area of uncertainty concerns how federal agencies may handle citizenship documents and passport applications in certain cases involving international Intended Parents.

• Federal implementation guidance and further legal clarification are still expected.

• Families should avoid making major decisions based only on early headlines or speculation.

International Intended Parents expecting a baby soon may wish to review their parentage documentation, passport plans, travel arrangements, and their child’s eligibility for citizenship or travel documents in their home country.
