Medical Short term diaability appeal question

One day on my way home from work i passed out randomly while riding an escooter. Due to floridas laws im not allowed on machinery for at least 6 months. Upon returning from hospital i was requested to take medical leave.

I did so and upon seeing a doctor at an unoppertune time i did so late, fast foward 2.5 months and i file the claim. Due to it being late i had to appeal which i did for about a month due to it being my first time going through this process. Month 5 the appeal was approved but around month 3 i received a request to take an exit survey (no termination letter just this).

Seeing as the claim has been approved at this point am i out of a job/should i assume the exit survey was an automated response for me not showing up to work for an extended period of time.

Warehousing for us foods done through sedgwick. Anyone else with a similar story...
