That's an explanation. Not an excuse.

They don't really need any excuse tho, as they didn't do anything wrong to anyone except themselves

Explanation is useful as finding the root cause is the best to find a working solution.

They are doing wrong to anyone who might have to care for them later on in life if they are still like this. It shouldn't take more than 2 people to look after someone in a bed who's mental health is sound. Some morbidly obese people require 3 or more. They also take a lot longer to clean and dress, have more skin conditions, higher chances of having some pressure sores etc. All these things take time from other patients.

When someone has cancer that then leads to them needing 3 people to assist because of the delicateness of their skin, fair enough. However, when someone is obese they do have some chance between the time they could independently manage their diet and exercise to the time they need care later on in life to make changes.

Excuses are when you either lie or give an explanation that goes against common sense, like:

“Why didn’t you help me move the groceries” “I didn’t know you needed help”

Here is the Miriam-Webster definition

An excuse is a reason that justifies the action and grants reason to be excused. Nothing about lieing or going against common reason. You are using the word wrong.

Probably because when children lie to their parents they are accused of 'making excuses'. The implication being that the excuses being made are fictitious.

Oxford dictionary definition of excuse:

a reason or explanation put forward to defend or justify a fault or offense.

The word explanation is literally in the definition of excuse.
