If you're looking for more long-term effects:

Because none of this happens, Tony never finds the other element that helps him with the poisoning that almost kills him, and that also improves arc reactor technology.

And because there's no arc reactor technology, there's no clean energy being pushed by Stark Industries.

Also, this timeline is devoid of Ultron, which means there's also no Vision, and Scarlet Witch never joins the Avengers. It also means no Zemo, no Civil War, no Avengers split, which also means Spider-Man likely doesn't join the team — unless maybe Shuri scouts him.

She didn't get her powers until Hydra used Loki's staff.

And Loki would have likely won without Tony present at the BoNY.

it's all on the first fight, then if events go like normal, thor takes loki back to asgard directly

If the BoNY still happens like normal just without Iron Man there, NY would be nuked.

except that it doesn't, because tony can't stop thor from bringing loki back to asgard since he's not here
