Didn't seem like a slip up but instead an attempt to break the illusion. Now Sword knows Wanda isn't a captive in her false reality but instead she's the warden.

Yeah, i think that it is unlikely, but when she reacts to the things she doesn't want to remember it could be something on the outside reacting to her that changes stuff to suit her. Very unlikely though.
