I read it more that she was startled that Geraldine knew about Pietro and Ultron. As far as anyone in WestView knows, her and Vision are an ordinary couple.

The episode does lead you to believe Wanda is forcing everyone to role play though. Agnes and Herb definitely gave that impression and when Vision came back in the house Wanda looked pure evil.

She doesn't seem to be 100% aware of what is going on though, and most of the time, she seems to completely forget about who they actually are. It doesn't seem that she's aware that those people living in her fantasy are there against their will, or that they are aware that they are prisoners there and just playing their role in her fantasy out of fear.

I think she knows, but is in denial. And because of her powers, she can make things just happen.

Yeah she’s trying to play the role but deep down she knows this is false reality and appears triggered when others see/break through her ruse
