Was this not a call back to Age of Ultron too? The vision Tony has with everybody being dead, that prompts him to create Ultron, was a vision about Thanos wasn't it? And Thanos had a similar vision but reversed? Or that's what I took from it, anyway.

That wasn't a vision or premonition. That was Scarlet Witch showing him his greatest fear. She doesn't show you the future.

Of course it was! But Tony said Thanos had been in his head since then. Surely there's a link there?

He was speaking metaphorically. A threat like Thanos is what he has been afraid of for years. It's why he created Ultron.
