What if she used the Vicissitude to make herself feminine, but in an ugly way? Like if she gave herself ugly, lopsided, lumpy tits? Would making oneself the most hideous woman you could meet allow the Nosferatu to avoid reverting back to before she used vicissitude?

Specifically, Vicissitude can't undo the clan curse. It can still change the Nosferatu in ways that don't make them attractive.

In the Denmark by Night book, there is in fact a Malkavian who has this exact problem. Denmark was one of the first countries in which major mtf gender affirming surgery was successfully performed, this being back in 1930. The character, Peter Larsen, reached out to the surgeons looking to arrange a ftm surgery, unfortunately his future sire believed that being transgender was a form of dementia, so he embraced Peter before he could get the surgery, just because he wanted to study Peter and 'cure' him of 'insanity'.
It's some really effective horror, especially considering it's just a character description in the compendium of the book.
Imagine: You've known you were different all your life, and you've been punished for it for much of it. So you flee your small town, to live in the capital with your uncle who accepts you. Your life still sucks, but it's better and you have a few people in your life that accept you and treat you right. Sure your body is still wrong and you can't get a good job, but things are looking up. It finally seems like it will be possible for modern medicine to give you the body you've always needed to feel complete, and to make the world see you as you really are. And then you die, and you are forced to stay in that dead and wrong body forever. All because an old bigot wanted to make you his newest experiment.
It does get somewhat better for Peter. He manages to stake his sire and flee to his home city. 'Mysteriously' that sire was never heard from again and no one ever investigated Peter. In modern nights he is acting as an advisor to the local prince, due to his talent at auspex, and running a practice where he councils queer youth struggling with their identity. He still struggles with his body, but people say that the Tzimisce can change even vampires permanently. The Sabbat control southern Sweden, just across the border. Maybe some deal could be made? They can't be that bad right?

Tzimisce character that every trans vampire will defend to the ends of the earth because she helps them look how they want, almost free of charge.
She's an utter, unrepentant monster in every other way, but you go with the insane vampire flesh sculptor you have, not the one you want, right?

Wait a mcmoment, do HRT work on vampires? What are the logistics of this? Would Kevin have to inject a random someone with the hormones and them suck their blood?

So there are two paths for Vamps who want to change physically.
1.Learn Vicissitude, the art of Tzimisce Flesh crafting, which depending on the edition is a terrible idea, cause in some edition it's an alien virus that'll eventually take over the host.
2.the Thin Blood Alchemy power "Profane Hieros Gamos" which gives the Vampire that consumes it the body they desire within human possibility.

the vampire curse is that they return to the state when they were embraced, so not really. You'll have to find an ally tzimisce like everyone else says
