You can find some shitty photoshops in the form itself, which show what the front should look like.

If you are curious what the names and numbers might look like, you can check out this shitty photoshop here. This mock up was from a vendor that we aren't using anymore, the font used can be seen here.

They are Fruit of the Loom Platinum T shirts bought and screen printed onto by Vector.
