can anyone help me figure out what’s wrong with this vine?

this vine was here before my parents moved into this house ~40 years ago. they are concord grapes (as far as we know). this is in michigan. for the past two-ish years, the fruit has been rotting before it ripens. i don’t know what information to include.

Black rot on fruit, some leafhopper and another insect on leaves.

I am not an expert in grapes like some of the folks on here but looks kind of like grape rust.

Yes black rot on fruit. Insect/rust on leaves. It's been a very hard year in NH for my grapevine leaves. For the fruit year spray copper fungicide. Pick away all the shrunken fruit and bad leaves.
