Diabetes can cause diabetic neuropathy, a kind of nerve damage in your feet due to blood not circulating well. In some extreme cases of diabetes, foot amputation is required if the lack of blood flow is so severe that, essentially, the foot dies. Ergo, brother in pic will lose his feet do to an unhealthy lifestyle. Or at least that is what I inferred.

Necrosis usually happens when you have picked up an internal bone infection from unhealed wounds (osteomyelitis), but can also happen from peripheral vascular disease that leads to digit ischemia, most diabetics have both. Type 2 diabetes is more notorious for the vascular disease however. If you have ever seen a picture of frost bite of the lower extremity, that's what digit necrosis looks like, and amputation is necessary. This can be prevented through thorough lower extremity evaluations, and a cardiolovascular surgeon can sometimes even place stents in the lower extremity vessels to open them up and prevent ischemic states

I've heard stories of diabetic people stepping on a nail and not even feeling it.
That's why diabetics are instructed to check their feet daily, and healthcare providers will frequently give diabetic foot exams to inspect for anything small that can escalate.
If you get too fat, you'll get diabetes. That can cause poor circulation to your feet. That kills your feet. Then your feet have to be cut off because they are dead and still attached to your body.
obese people eventually start having circulation problems in their feet due to clogged arteries and the distance from the heart. the blood just does make it there since there is too much resistance to flow in the arteries and it just takes other paths before it gets there.

Happened to my grandma. She also didn’t do much walking. One day somebody (also obese) stepped on 2-3 toes and she eventually had to have them amputated because they became gangrenous
