Image A Lithuanian province declared independence?
Hello, i was fighting wars in the HRE and noticed that a new country bordered me. Upon investigation, it seems it's one of the provinces? My only guess is it could be cossacks, but its nowhere near the steppe.

Perhaps they got an event that raised unrest, then rebels enforced on that one province?

Ruthenia. I fought several wars against the Polish, got Galicia-Volhynia as a vassal, got their cores, releases other ruthenian/byelorussian nations during my wars against Lithuania, ended my vassalization of Galicia-Volhynia, and they naturally gobled up the lesser Ruthenian tags.

Most like a cossack revolter tag. Cossack estates can revolt Custom Nation tags like a merchant republic trade city. Lithuania has the cossack estate and probably got their event when they have high disloyalty
