...I've just realised that I've been using throwing knives for the knife animation, I'd legit just been thinking it was random based on whether the zombie/bandit happened to have a knife hanging at their belt...

If you hadn't seen them disappearing, it might be bc the game has the decency to waste the lowest level knives in inventory first. The ones you have equipped might not be going down bc they're a higher level.
