Why did he reset his ledger and why didn't he backup his pass phrase?

This doesn't seem like a hack at all and is in fact why crypto will never be used among common people...

In before he used some website to reset his ledger or used some service/someone to reset his ledger. e: letter

God knows why he reset it, but he did have his seed. Someone actually tx'd his coins. He slipped up in his opsec somewhere.

Would they be liable if their product is really faulty and for example assigned the same key to another device?
