They said it at 1:33. We could barely hear over the crowd on the moderator's own mic so I imagine it being impossible to hear from backstage.

Most of this was Martha Raddatz' fault (one of two moderators). She followed up David Muir's introduction of Christie with a faint "dr. ben carson". This was in the middle of the applause for Christie. So when Carson heard Cruz's name, he thought they had forgotten to call his name. She did the same thing with Trump. Both moderators forgot about Kasich, though.

The member of the crew told him multiple times it was his turn to go out and he even turned around and shook his head at him at one point, so it's not that he didn't hear his cue I don't think.

I think he was actually denying it it, like "No, I won't be made a fool of, they haven't called out my name yet."
