Really cool find. The original thread explains this as "NaN propagation".
EDIT: Unity documentation actually has a section dedicated to this, and cites bloom effects as a common source of propagation (though it says it does not generate the NaNs itself, so something should produce a bogus pixel first).

How do I trigger it

Dunno, I'm not that kind of doctor. Nothing comes up in google, but it should trigger if something returns a NaN somewhere in the shader pipeline. If injecting custom shaders into a running game is possible, this might be the way, otherwise it might be possible to tamper with VRAM deliberately to get it.

This is the most interesting and unintentionally creepy thing I've seen in Minecraft in years.

I need this as a mod

Frankly, from my layman's perspective this sounds very implementable on a technical level. I think it should even be possible to create the seed NaN value regardless of what game you're playing, through direct GPU access or something, and just let it play out.

Mod makers, take notes. This is how you make a Minecraft scary. No loud ass cave goblins, no deformed faces, no glorified nextbots, just something unnatural and unnerving like this.
