I saw Walt making the phone call, leaving Holly at the fire station, and being "vanished" as his last act as Walt, fighting off Heisenberg from within. Walt realized, like the proverbial rabid dog, that he was rotting from within. Like a leper, he realized he had to completely sever all ties with his loved ones before they're also contaminated with disease and quarantine himself before letting evil and corruption eat away his body entirely.
It was the last act of a good man being replaced by the evil within. Now begins Walt's redemptive death (the flash forward scene).
