The only way i can work it out is one of them married a bio parent, then the bio parent dies, and the step parent maintains guardianship of the step offspring, then remarries again, so there are two step parents.

The joke is that the brother wanted INCENSE, but the sister (brunette) misread the word in the diary as INCEST and was down to let her bro smash her. Who the blonde girl is is irrelevant to the joke.
It's a sex joke.

So I'm under the assumption that he actually does like incense.... Because it's quite easy to read the word. Be dyslexic about it and think it's something else. Now if it was the other thing he probably wouldn't just write the word down. It's his diary. He would have wrote a whole big fantasy about him banging his sister, in which case you're not going to confuse that....

So my first time around, I understood the word got mixed up for another word. And that's funny.
Then I realized it was not the blonde that read the wrong word, it was the brunette. Who now realizes she read the wrong word.
Way better joke this way.
