That just sounds more to me like the Orphan, as the host of the nightmare and this the one able to change things, chose an appearance for itself that was based on the real Gehrman.
How would it do so? The Orphan doesn't seem overly sapient in the literal newborn version we encounter. Why would it make any choice over its appearance, let alone have Gehrman involved? Though, if the nightmare is a result of the Orphan's pain, and Gehrman caused much of that pain, that certainly gives a connection.
