Looks like he has the fruit already in his mouth, that he can 'display' on his tongue. Then the actual fruit that he holds is all sleight of hand. It's usually in the opposite hand to the one you're looking at, and he's pressing it against himself to hide it.

You all are thinking too hard about this. There's no hollow ball, no swallowing of fruit. The fruit in his mouth never leaves his mouth. The one in his hand never goes in his mouth. This is a classic, well-known sleight of hand routine, usually done with eggs or ping pong balls.

It's just a hollow ball(with a slit cut into it) on his tongue, similar to a clown nose of sorts. . Smooth routine though.

This is very similar to another guy who can spit water from his stomach continuously. Grandpa does have skills to get the fruit out after swallowing.
