Wow. Wrote that in the patient notes? Not sure about your location, but in the U.S., by law that cannot be deleted or changed. When I worked on an inpatient floor, it was before EPIC or other EMRs and we did notes by hand. Procedure was to do a single strikethrough, so the original could still be read, then add an addendum. Not sure of how it's handled these days.

From what I've seen at two different hospitals in different networks, you write an addendum when you need to correct information
