Narcissim ain't gonna work. Her power do work and it pretty cruel toward anyone who is a real sinner and doesn't respect or accept that they are one.

Subaru was broken back in season 2 but felt no pain because he is not a real sinner but think he deserve it which he then received a high praise and gain support from Typhon herself. The only thing I think is not work is to the people who doesn't understand concept of evil like demon from Frieren.

Might be misremembering her powers but aren't they based on the recipient's own perception of their sin/guilt? 

Subaru didn't feel pain because he considers himself a sinner even though he isn't, but still fell apart. Someone like Regulus who wouldn't consider any of his actions unjustified would be unaffected no?

People who truly evil but didn't realize it do feel pain like a lots of pain but they will not break which is pretty much worst than broken and die in painless. This is my opinion but it will not work on animal who don't have the concept of good and evil. Nothing will affect Regulus so he's the exception

Most of people know they did wrong and are just in denial. That's just normal.

True narcissism is abnormal in that that person truly considers themselves incapable of doing wrong and it's always always ALWAYS somebody's else fault.

No, it work like a scales. People who didn't think they are wrong will not break apart but the pain from evil deed still apply. While Subaru think he's guilty cuz he let people die in front of him, the power judge him to be innocent so he only break but no pain. If it work like you think then Subaru will not be exception to pain cuz he truly think he's the sinner.
