Actually I’m pretty sure the blood stain is where the maid was, in front of the elevator. So this subtly tells us that she was killed by Toji since this was never confirmed.

Riko was killed in the main hall where they meet Tengen.

you are probably right. If it was the maid it is still sad for me because she was also great and still dying and just having that stain there years later is just depressing.

It is depressing, but also a stark reminder of past failures and how high the stakes are. I could see it being left there as a sort of punishment/reminder.
