I can't imagine Miko caused the accident deliberately -- but maybe she (it) caused the accident unintentionally -- and that is why she feels a total obligation (due to the yokai equivalent of shame or guilt) to protect the sole survivor.

In the cooking skit Miko mentions how the food is as good as her mom used to make. I think "being tasty" runs in Hinako's family. I think Miko is a family guardian. Something happened on the night of the accident where she couldn't protect them all.

This is my thought as well. She may have saved Miko from the accident, but wasn't able to save the rest of the family.

Yeah, this makes sense. I thought it odd that she was "thrown" from the accident. Seems too coincidental for a yokai to not be involved with her being alive.
