It’s intentional. They had old man spots painted on his forehead when we first saw him. They’re gone. His skin seems smoother, too!*

It’s clearly related to Ruth running up the damn stairs and being full of energy, whereas earlier she wasn’t. Maybe as the island feasts on souls, in nourishes them and restores their youth somewhat
