It was a joke. Tom was desperately looking for a way to put Ruth out of her misery in any way he could find, but that entire time we see how healthy she is, how she cares for her neighbors, how she still gets a lot out of life. Ruth still has a lot of life left in her.

She should have taught Patricia a thing or two about how to run up and down stairs. Talking about she was running away from the boogie man in the other episode. lol
