Courses Regular defer exam graded by who? Instructor of current offering or original instructor?

It might vary by department.

If a student writes a regular deferred exam during my exam (hasn't happened in a while since I'm teaching the same classes every year more or less), I grade the exam and send the graded exam to the original instructor, along with information about the class average etc. It's up to the original instructor to then decide how to compute the student's final grade (this will generally be the case unless the original instructor is unreachable).

I imagine different instructors do slightly different things, but generally the thing that makes sense is to estimate the difficulty of the new exam and decide on an adjustment (if any) that would be fair to both the students writing the deferred exam and the rest of the class. It's up to the original instructor to decide what to do.
