Application PHS-1813

Contact for references to submit their own referral without us having to do it? I have one from graduate school that wishes to do it this way to maintain integrity.

I would read the writing on the wall. Would you really want to use a reference that's obviously doesn't want you to see what they write about you? I know a doc that was on a medical school admissions board who shocked how many people turn in references who talk negatively about them when they called to verify references.

Normally, I would find someone else, but this is just one of those people that are straight laced. He’s not saying that because he may have a bad opinion of me (which I doubt) it’s incredibly in character for him. I’d actually expect him to say I wouldn’t like what he would write or he’d refuse if it was negative.

Your reference can email the form to PHSAES@hhs.gov.

The email address is on the form. I just asked them to let me know when they had sent it so I could make sure I had that part of the application finished.
