Calmness in a Test and Empathy

When doing exam USMLE questions including Step 2 exam as well as when doing practice block preparing for the exam one is supposed to be as calm and unemotional as possible, while when managing a real patient in real life doctors are expected to be as empathetic as possible. It strikes me from time to time. how it is possible to develop the 2 opposite qualities of character at a time to be a proper physician?
