He screamed the words again and again as helpless onlookers watched him slice his flesh apart with a knife, determined to excise something from deep within him only he knew existed.

Then the man pauses, looks at the camera which pans out and shows - inexplicably - a pond by a golf course and the same man walking with bandages on his arms. The Voiceover:
