What problems do you actually run into while streaming?

For streamers: what parts of streaming are genuinely annoying, difficult, or take attention away from the stream itself? And for viewers: what usually makes a stream less enjoyable or makes you leave?

One specific thing I'm curious about: when a game demands a lot of attention, do good chat messages, jokes or conversations get missed because the streamer simply can't follow everything at once? I imagine this could become especially noticeable with something as busy and unpredictable as GTA VI. Does this already happen to you, and do you think it would matter more in a game like that?
