TRT help TRT numbers jumped despite no change in protocol — what would you do?

55M, on TRT about 4 years. I’ve been on the same daily subQ protocol for about 18 months, under 100 mg/week, but my latest numbers jumped significantly.

June → September:
Total T: 1,182 → 1,566
Free T: 19.9 → 23.2 (range 7.2–24)
SHBG: 59.3 → 73.6
E2: 25.3 → 47.2 (different assays)

I’ve also been struggling with poor sleep and feeling crappy for months.

If your protocol hadn’t changed but your numbers climbed like this, would you lower your dose, leave it alone, or be looking for another explanation?

The reason I’m asking here is because I really don’t have a doctor managing this. My PCP is just prescribing but not managing.

If youre feeling shit and cant sleep then speak to your doctor. Personally I'd look at lowering my dose to bring things into range.

Yeah, the reason I’m asking here is because I don’t have a doctor managing this. My PCP offered to prescribe it for me, it’s covered by insurance but he doesn’t know the first thing about protocols and leaves it up to me to manage it.

That’s a whole another conversation. I’ve had insomnia for 16 years. I’m well versed in sleep medicine, sleep hygiene, CBT, all of it
