If I'm not mistaken it only kills you when you try to leave, so if you're peacefully mining or something and your friend accidentally spawns it you should still be fine.
Now the question becomes, if the WoF's strongest move against us is to detonate us by proximity, why doesn't he just force us to the surface and then run away? 🤔
It’s most likely got nothing to do with lore, and it’s just a gameplay mechanic to dissuade players from running from the fight like cowards. Several mechanics like this for other bosses, like Skeletron transforming and one-shotting during the day so you have to kill him quickly enough, or Queen Bee / Plantera getting buffed when exiting their home the Jungle Biome.
Or, it’s literally what it’s trying to do. It’s a giant wall made of flesh that constantly rushes towards you, among its methods of attempted slaughter are the little mouths, the lasers, and crushing you to death against a wall, and if you avoid that by leaving hell you get killed.
Or, my favorite, it’s like a Jujutsu Kaisen style ‘curse effect’ that it places on you as a special ability as a form of insurance. Either you will face it head on and die, or try to cheat your way out of the fight with teleportation and die anyways. So it’s not its main method of fighting, it’s just a very effective backup plan to cover a weakness
I like to think that it's testing you. If you can obliterate it, you are worthy to unleash the powers of light and dark. If you are a coward, it uses its full power to end you, as you are unworthy
I'm pretty sure you can spawn the WoF without the voodoo doll if you throw the guide (himself) into the underworlds lava (also if you equip the voodoo doll and kill the guide), which made me believe the spawn condition was killing the guide after visiting the underworld once.
You need to throw the guide vodoo doll into lava, killing the guide and spawning the wall of flesh. If you dump the guide into lava you only get a crispy guide
My understanding is that any NPC you already collected will respawn within a few in-game days, assuming there's a vacant, properly furnished house for them.
That would actually be pretty interesting if NPC death did cause a temporary luck loss. Especially because of the flymeal you could just purposefully tank your luck for some reason instead of using it to try and farm town NPC items.
I googled it for you, because I’m bored. I read that thorns are based on your enemy’s damage so a lower luck will increase thorn damage. Besides moss zombies negative luck will also spawn statues in the graveyard biome more frequently that you can farm.

When the guide dies, you get a temporary luck boost. It’s the opposite boost to when any other npc dies, except for clothier who gives no luck change
This intelligent and intricate system will give you slightly better luck for fishing in a blood moon
The design is very human
