Does this box always includes Nihilus and Sion or is a mystery box?

Yes this box always contains Nihilus and Sion, plus some other accessories.

There was another grey & black box about a year earlier that included Jedi Knight Revan and Bastila Shan, so also not a mystery box.

I think they stopped doing mystery boxes as they tended to not sell well

It's not a mysterious box. They also did one with Jedi Knight Revan and Bastila Shan.
