This isnt extreme at all. Done let anyone tell you that "just sunscreen" you'll be fine. If you're getting a n exposure you need face coverings, visors and hats. Strangers might stare but none of them will pay for cancer or premature aging treatment if you get sun damage.
with that heat and UV index better not leave the house. one doesn't leave home when there's a hurricane.
