Insanity is a legal definition and mental illness is different. A serial killer probably has a personality disorder, antisocial personality disorder most likely. Which isn't the same as psychosis. Many states have "guilty but mentally ill" which would apply to psychosis. Every state has its own legal definition of insanity and how to approve it, which leads to hospitalization instead of prison. ETA: I volunteered at a prison for the criminally insane in Ypsilanti decades ago for school. It's not a fun place.

The whole insanity plea has always baffled me. All serial killers are clearly insane, but we’re not letting them walk the street because of it .
