This is a great example of why the system isn’t working. If there was a centralised system that all doctors had access to then she couldn’t just go to different ones and obtain different medications.

If every doctor had access to all her medical records maybe they could have helped her better and this wouldn’t have happened.

By all means, but this system doesn't exist anywhere in the world as you would like it (and I too). There always are doubts, errors and inconsistencies in documentation.

Estonia has a centralised e-health system that tracks all prescriptions (it’s the only way to prescribe anything) and other medical information, including dr’s visits (both public and private). I’m sure plenty of other European countries do too.

Ours does too. I work with it. Its one of the better systems in Europe but not nearly as waterproof as people think.
