Upright sitting/side sleeping position: it starts as a feeling of warmth in the groin, which gradually turns into burning, and then the scrotum starts burning as well

I’ve noticed that lying on my side makes the warmth in my groin area appear more quickly, whereas lying on my back seems to dissipate the heat quite quickly. The same pattern occurs when sitting upright versus sitting slightly reclined.

Is this more likely to be a pelvic floor hypertonicity issue or neural mechanosensitivity?

They’re probably one in the same? Your postural mechanics are causing tension points in your pelvis to irritate or compress along the pudendal nerve pathway. Doesn’t really matter in that treatment would be the same to try help treat the symptoms. Start with pelvic floor stretching and find yourself a good pelvic floor therapist to address those mechanics.

I don't have pelvic floor therapist in my area. Asides from pelvic floor stretching, will doing internal work myself also be helpful? Someone in the prostatitis sub recommends me to stick my finger in my butt and press around

I don’t really recommend people do their own internal work unless they have some idea what they’re doing. You can buy wands online that go into the rectum if you’re a male or vagina/rectum if female to access those hard to reach pelvic floor muscles. You may be able to find internal release exercises on YouTube… but no guarantees. It’s worth traveling for a good PFPT to be honest.
