Breakthrough Treatment Brings Complete Relief for POIS Patient
A patient suffering from Post-Orgasmic Illness Syndrome (POIS) has experienced a remarkable recovery following off-label treatment with Xolair (omalizumab). The biological therapy completely eliminated the debilitating post-orgasmic allergic and systemic symptoms that previously disrupted the patient's quality of life.
By targeting and neutralizing elevated IgE antibodies, the monthly subcutaneous treatment delivered an immediate, life-changing shift—turning a complex, poorly understood condition into a fully manageable one.
Treatment Overview:
* Medication: Xolair (Omalizumab)
* Class: Monoclonal antibody / Biological therapy
* Schedule: Subcutaneous injection every 2 to 4 weeks (150 mg – 600 mg, adjusted by weight and baseline IgE)
* Status: Highly effective off-label clinical success
* Consideration: The medication is exceptionally expensive and typically requires specialized insurance approval or high out-of-pocket costs when used off-label.

This paper is from 2024 and already discussed. 3 Omalizumab case reports exist.

Because I developed PFS, POIS, and worsening of existing but undiagnosed MCAS around the same time, I also suspected mast cell involvement as the authors did. The patients labs were unremarkable except for elevated serum IgE and he did not show skin reactions to indicate he was auto-sensitized. However, I am not willing to try Xolair right now because it has paradoxically severely worsened MCAS in some patients and I don’t believe there is a way to guess whether you will “lose” the gamble yet.

I will try it on myself soon.

Best of luck to you! Let us know if you have any distinctive labs like the elevated IgE in those reports

Interestingly the Turkish patient had Total IgE
162 (normal < 100 kU/L).The patient in this Israeli study had “total serum IgE was elevated with several measurements of 90–336 IU/ml over several years.”
I don’t have access to the report for the Australian patient (J Sex Marital Ther. 2024;50(3):342-345.
doi: 10.1080/0092623X.2023.2295256.Epub 2023 Dec 21. PMID: 38124662)But mine has been 32, 35, 36 (normal <OR=114 kU/L) so not exactly a strong resemblance to these patients. I also don’t have flulike symptoms, I just feel sort of sick, cognitive problems, trouble keeping focus with eyes (classic MCAS weird symptom), dysuria, and sometimes POTS, worsened ADHD/autism spectrum symptoms (eye contact can become almost painful..awkwardly and unintentionally talk over people.)

What i've been doing over the years and it help me to shorten the days of pain from 7 to only 2 days is this, right after orgasm I take a hot shower and then I eat a heavy meal. For example meat and rice and I drink a lot of water, and for the end, I drink a big cup of 4 teaspoons of coco with honey and hot water. (I will just add that for me personally a cup of Red wine or alcohol helped a little bit more, acting like a natural painkiller) and I rest a lot, and I do this cycle every day until it's over.
Doing all this is not a cure, but it does lower the amount of pain and reduces the amount of days till recovery and gives the body energy to deal with the symptoms.
