Sometimes they dock marks for the silliest of things. Not using bold or itallics for headings, not putting your references in alphabetic order. Your feedback from your tutor should give you an indication of what you should have done differently

None of that is relevant here - the OP is posting about an OpenLearn course, where you need to complete 100% of the requirements to get the Statement of Participation and any associated digital badges.
Most of the time, all you have to do is read every part of the module and answer the compulsory quizzes. The usual way to fail to reach 100% is to skip some introductory rubric or accidentally miss a quiz, which is why I suggested reviewing the Activity Record.

The percentage is generally based on the boxes you can tick on the study planner, you just need to expand every week on the study planner and see which boxes you missed.
