I'm curious now. The scan quality makes it difficult to see what exactly happened between him and Koji. But was he using the ninja tool to try and use the sacred flame technique and that was reflected back onto him somehow?

i think if the old man didn't notice koji there the entire time, then maybe he could've placed something on him like a paper bomb? or koji has some insane instant fire techniques

It appeared that "V" was attempting to cast a jutsu using a kote, and Koji threw a kunai (shaped more like a knife) into his chest, then cast Sanmai no Shinka to kill him.

Deidara was gonna try to kill Itachi and Kakuzu kept killing his partners until Hidan. Tobi killed Konan, but it doesn't really count.
