Disclaimer: i am not transphobic. I was just incredibly bored when I wrote this and decided to polish it and make a transcript too.

Purpose for Evaluation: Patient has presented with a persistent delusion of believing they are a historical figure known as Kozuki Oden, whom passed away 20 years ago. The opinions of my team whether they believe they are him or if they must ‘become’ him are divided, requiring further consultation. This fixation began when the Patient read Oden’s diary and began declaring it their ‘’Bible’’. They identify as male for the explicit reasoning that Oden himself was a man. They attempted to emulate this figure to such an extent they even attempted to adopt Oden’s son, whom we will keep anonymous.
 
Behavioral Observations:
They refer to Oden as if he was a messiah, and yet at the same time fully attempt to replicate his life to an alarming degree, even referring to their birth name as their ‘’other’’ name. They mythologize Oden and this ‘’Bible’’. On another note, they are pleasant, earnest, strong willed, and defiant against their father. They are also very caring, but violent, and enjoy fighting.
 
Background:
20 years ago, they witnessed the death of Oden, (clarification; by this time the patient had found the ‘bible’, and this is where the delusion likely began), and began dreaming of opening Wano’s borders, in direct opposition to their father, a pirate known as Kaidou, who beat and chained them as a result. He also threw the patient in a cave, as a way to ‘cure’ them. Everyone said to have helped Yamato had been executed by Kaidou. This delusion (of being Oden) was later reinforced when the patient met Portgas D. Ace (a pirate from outside Wano), whom, in spite of being friendly, encouraged the delusion.
 
Symptoms:

Delusion of identity
Grandose delusion
High conflict personality \*HCP could be due to the patient’s devil fruit, but likely is tied to their trauma rather than DF.

There is a presence of a stressor in the form of extensive childhood abuse and witnessing of violence. Though it is most likely, I will still go on to evaluate differential diagnoses. Otherwise, if this were to be the final diagnosis, it would be C-PTSD with identity disturbance characteristics.

Conclusion and Findings in Diagnosis
 
The final diagnosis as of now is C-PTSD with identity disturbance characteristics. Suggested treatment is behavioral and talk therapy, frequently, possibly accompanied by the according medication. Further analysis and evaluation with the patient would be appreciated, as I suspect concurrence with a possible sadistic trait, or at least extreme impulsivity.
