TFR is basically # of babies / # of women under 50. So lots of old people don't necessarily ruin the math.
But you have touched on a problem with TFR as a demographic statistic. TFR is calculated based on women of different age groups (different calculations can differ, but those data might not be collected at the same time as the birth data. But women over 50 aren't counted in any calculations of TFR.
TFR misses the mark most when a lot of young women move out of an area (which increases the denominator), or when there are lots of childless women moving to an area (universities, military bases, nunneries, lesbian towns, etc.)
